Description & Project Overview
The Sulu Sanitarium and General Hospital - Bids and Awards Committee intends to procure VARIOUS MEDICAL SUPPLIES through Small Value Procurement pursuant to Section 34 of the Implementing Rules and Regulations (IRR) of Republic Act (RA) No. 12009.Please quote your best offer for the items described in the Request for Quotation (RFQ) form, subject to the Terms and Conditions provided therein. Kindly submit the duly accomplished RFQ via e-mail on or before November 6, 2026. RFQs not submitted in due time shall be rejected.The following documentary requirements are also REQUIRED to be submitted together with your accomplished RFQ form:1. Valid PhilGEPS Certificate of Registration (Platinum membership);2. Valid FDA License to Operate (LTO);3. Notarized Omnibus Sworn Statement in GPPB-prescribed format (Revised); and4.
