Contract Description
The subject of the order is the supply of equipment for ablation procedures along with the lease of devices for the needs of the Interventional Radiology, Hemodynamics and Electrocardiology Laboratory together with the Electrophysiology Laboratory of the Provincial Hospital in Kielce. A detailed description of the subject of the order and the quantity is specified in Annex No. 2 to the SWZ - assortment and price form. The technical and functional parameters for the subject of the lease are specified in Annexes No. 2a and 2b to the SWZ. Information regarding the principles of delivery, guarantee and remuneration is included in: - Annex No. 3 to the SWZ - draft provisions of the public procurement contract on the basis of a deposit - Annex No. 3a to the SWZ - template of the lease agreement
