The subject of the order is the purchase and delivery of the drug Lisocaptagene maraleucel used in Cat-T therapy for the Hospital Pharmacy of the Świętokrzyskie Oncology Center in Kielce. The subject of the order is described in detail in the Assortment and Price Form constituting Annex No. 1a to the SWZ. The Ordering Party requires that the offered medicinal product be included in the lists in the current, as at the day of opening the offers, Announcement of the Minister of Health on reimbursed medicines, foodstuffs for particular nutritional uses and medical devices and its price should not be higher than the applicable financial limit of medicines used under the B.12.FM drug program. The offered medicinal products must be admitted to marketing in the territory of the Republic of Poland in accordance with the principles specified in the Pharmaceutical Law Act. Detailed terms and conditions of the order are described in Annex No. 1b to the SWZ.
