1. Detailed description of the subject-matter contained in this SWZ and: 1) annex no. 1 offer form; 2) annex No. 2 form of assortment of the two-stage offer form at the same time as the description of an item of supply; 3) annex n° 4 project agreement. 2. All the requirements laid down in the SWZ in the documents indicated above lay down minimum requirements, and their fulfilment is compulsory.Specifically in the description of the objects of medical use: For medical work: 1) the current document confirming the use of medical work as a medical work which meets the requirements of the Ustawy z dnia 7.04.2022 r. o wyrobach medycznych (t.j. Dz. U. 2024 r. poz. 1620) and zgodność z Rozporządzeniem Parlamentu Europejskiego i Rady (UE) 2017/745 w sprawie Wyrobów Medycznych (MDR) / (EU) 2017-746 w sprawie wyrobów medycznych in vitro, posiadający oznaczenie wyrobuznaki znakiem CE - t: deklaracja zgodności i/lubertyfikat wystawy przez producentów upoważnionego/autorywanego przedstawiciela potenzawiańskiego), iżwanych jest zgodnym z wymaganiem jednostkowych zasadnic; 2) not identifying identities (t)For biological products: 3) the authorisation of the Minister of Health or of the President of the Office of the Registration of Biological Products, Wyrobów Medycznych i Produktów Biobójczych i obrót produktem biobójczym zgodnie z przepisami ustawy z dnia 09.10.2015 r. o produktach biologicwych; 4) information materials (prospectus, catalogue, card, form of data, part of the instructions of the manufacturer, declarations of information materials describing the objects of production and its parameters, etc.), authorizing the requirements for biological wastes, 5) declaration of potency, 2.2.Wikonawca może uzupełnić przedmiotowe dowodowe, ale środki tylko nawanie wezwowiającego, provided that the Wykonawca does not deposit the objects of the dowodowego środka lub złożonowego przedmiotowowe środki dowodowie będą niekomplete.
