Pakiet nr 15 Risankizumab B 47. Szczegółowy opis przedmiotu zamówienia stanowi Załącznik nr 3 do SWZ – Formularz asortymentowo-cenowy. Zgodnie z Rozdziałem III pkt. 3.SWZ: Warunki przedmiotowerequired from Contractors - Contractors submitting an offer in the procedure declare that they undertake to comply with the following requirements: 1) They will offer medicinal products that meet the requirementsspecified in the Pharmaceutical Law Act of September 6, 2001 (Journal of Laws 2026.612, consolidated text of May 2026, 2001), in accordance with the description of the subject of the order contained in the assortment and price formsstanowiących załącznik nr 3 do SWZ. 2) Wykonawca posiada dokumenty potwierdzające, że produkty wymienione w Formularzach asortymentowo-cenowych stanowiących załącznik nr 3 do SWZ, są zarejestrowane wUrzędzie Rejestracji Produktów Leczniczych, Wyrobów Medycznych i Produktów Biobójczych zgodnie z wymaganiami ustawy z dnia 06.09.2001r. Prawo Farmaceutyczne (Dz.U.2026.612 t.j. z dnia 2026.05.07). 3)Dla leków we wszystkich pakietach 1-30 Zamawiający wymaga ważnego zezwolenie GIF na prowadzenie obrotu produktami leczniczymi. 4) W pakietach 1-30 znajdują się leki biologiczne do programów lekowychThe ordering party requires that the drugs be approved for use and reimbursed under drug programs. The entire range consists of medicinal products B. The Contractor will sign annexes to the contracts ifthe refund for the duration of the contract will change to products (with the same international name) that will be covered by such a refund. C. Price of the drug in the case of a drug covered by the official price duringthe entire duration of the contract must be consistent with the current official price of a given drug on the day of order execution, in the event of a change in the principles of financing the purchase of drugs by the National Health Fund, lowering prices orreducing the official price of the drug below the price resulting from the offer. Contractors will sign the annex for the duration of their validity, after this period the price will return to the price resulting from the original tender offer.Contractor, which is always the maximum price for the Ordering Party. D. The product will be transported to the place indicated by the Ordering Party in accordance with the requirements set out in the CHPL.medicinal products. E. The Contractor will provide the Summary of Product Characteristics (ChPL) or its equivalent translation into Polish (PACKAGES 1-30) with the offer. 5) The Contractor will provide, at his own expense, iryzyko transport produktów leczniczych do miejsca wskazanego przez Zamawiającego. Zamawiający ma prawo do skontrolowania środka transportu, którym przewożone są produktylecznicze pod kątem obecności zapisu temperatury panującej w komorze transportowej, obecności podestów, na których przewożone są pudełka z produktami leczniczymi. W przypadku niespełnienia wymogówtransportu przewidzianych w aktualnych dokumentach potwierdzających dopuszczenie przedmiotu zamówienia do obrotu przewidzianych Ustawą z dnia 06.09.2001 r. Prawo Farmaceutyczne (Dz.U.2026.612 t.j. zdnia 2026.05.07), Zamawiający ma prawo odmówić ich odbioru i poinformować o zaistniałym fakcie Wykonawcę. Wykonawca zapewnia transport dostosowany do infrastruktury Zamawiającego. Transport wszystkichproduktów leczniczych będzie odbywał się odpowiednimi środkami transportu, gwarantującymi utrzymanie prawidłowej temperatury dla przewożonych produktów leczniczych i wyrobów medycznych posiadającymithe possibility of heating or cooling the transported goods and compliance with GMP rules, if applicable. 6) Offer products with an expiry date of at least 12 months from the date of delivery. 7) During durationcontract, the Ordering Party has the right to request the Contractor to present current marketing authorization documents, safety data sheets, leaflets - in Polish. 8) They will offer a payment date of 30dni od dnia dostarczenia Zamawiającemu prawidłowo wystawionej faktury VAT. 9) Podpiszą umowę, której wzór stanowi załącznik nr 4 do SWZ. 10) Zaoferują maksymalny czas realizacji zamówień „cito”: 24hours, and in normal mode up to 48 hours from placing the order, regardless of the value of the order placed. a) In the case of on-call deliveries, when the delivery date falls on Saturdayor on another public holiday or outside the Ordering Partys working hours, the delivery will take place within the agreed delivery time, i.e. a maximum of 24 hours from placing the order to the indicated place.by the Ordering Party. b) In the case of orders in normal mode, the deadline for which falls on a Saturday or a public holiday, the deadline for execution expires on the first business day afterthe date specified above, but no later than 11:00 a.m. on that day.
