Poland – Road transport services – Usługa transportu sanitarnego dla Szpitala Powiatowego w Chrzanowie -Sprawa nr: 85/2026

The subject of the order is the provision of sanitary transport services for patients of the District Hospital in Chrzanów by appropriately qualified personnel, using appropriate means of transport, in accordance with the needs of the Ordering Party. The order includes sanitary transport performed by a sanitary (rescue) transport team consisting of two people: a rescuer, a driver-rescuer, in particular a patient from the hospital to another medical facility and back, transport to and from the place indicated in the order. The service is provided on call 24 hours a day, 7 days a week. 1. Communication in the contract award procedure and competition, including the submission of offers, applications to participate in the procedure or competition, exchange of information and transmission of documents or statements between the ordering party and the contractor, taking into account the exceptions specified in the Public Procurement Law, is carried out using electronic means of communication. Means of electronic communication are understood as means of electronic communication as defined in the Act of 18 July 2002 on the provision of electronic services (Journal of Laws of 2019, items 123 and 730). 2. The offer and declarations referred to in Art. 125 section 1 of the Public Procurement Law, subjective means of evidence, powers of attorney, and the obligation of the entity providing resources are prepared in electronic form, in generally available data formats, in particular in the .txt, .rtf, .pdf, .doc, .docx, .odt formats.
