1. The subject of the order is delivery to the Provincial Hospital in Bielsko-Biała at Al. Armii Krajowej 101: 1) lenses, viscoelastic fluids, kits for cataract surgery and posterior vitrectomy with the use of a device for phacoemulsification and anterior vitrectomy, a device for posterior vitrectomy and phacoemulsification and an optical biometer - Part 1; according to the quantitative and type list specified in Annex No. 2 to the Specification of Terms of Order, hereinafter referred to as the SWZ, meeting the quality parameters specified in Annex No. 2 to the SWZ, under the conditions specified in the draft provisions of the contract constituting for Parts 1-2 - Annex No. 5a to the SWZ, for Parts 3-10 - Annex No. 5b to the SWZ. 2. The goods offered must be new and unused (except for loaned devices for eye surgery). 3. Delivery of the subject of the order, including transport and unloading, will take place at the expense and risk of the Contractor. 4. Applies to Part 1 - For the duration of the contract, the Contractor is obliged, as part of the price of the subject of the order in the scope of Part 1, to provide, together with the first delivery of lenses, nine reusable implantation sets consisting of an injector and tweezers for implanting lenses specified in item 1 and two sets for marking the eye during toric lens implantation. 5. Applies to Part 1 - For the duration of the contract, the Contractor is obliged, as part of the price of the subject of the order in the scope of Part 1, to deliver and deposit at the Ordering Partys office within 7 weeks from the date of conclusion of the contract, a device for phacoemulsification and anterior vitrectomy, a device for posterior vitrectomy and an optical biometer with the parameters and instrumentation specified in the table below, and ensure the efficient functioning of the devices throughout the duration of the contract. All costs of operating the devices, including inspections, repairs, maintenance and servicing as well as training of the Ordering Partys staff in the use of the devices, must be included in the price of the subject of the order in the scope of Part 1.
