35/26 – Delivery of lenses, sets and accessories along with the loan of devices for eye surgeries Part 2 - Delivery of accessories for antiglaucoma surgery along with the loan of a device for antiglaucoma surgery
1. The subject of the order is delivery to the Provincial Hospital in Bielsko-Biała at Al. Armii Krajowej 101: 1) accessories for anti-glaucoma surgery with the use of a device for anti-glaucoma surgery - Part 2; according to the quantitative and type list specified in Annex No. 2 to the Specification of Terms of Order, hereinafter referred to as the SWZ, meeting the quality parameters specified in Annex No. 2 to the SWZ, under the conditions specified in the draft provisions of the contract constituting for Parts 1-2 - Annex No. 5a to the SWZ, for Parts 3-10 - Annex No. 5b to the SWZ. 2. The goods offered must be new and unused (except for loaned devices for eye surgery). 3. Delivery of the subject of the order, including transport and unloading, will take place at the expense and risk of the Contractor. 4. Applies to Part 2 - For the duration of the contract, the Contractor is obliged, as part of the price of the subject of the order in the scope of Part 2, to deliver and deposit at the Ordering Partys headquarters within 4 weeks from the date of conclusion of the contract, an anti-glaucoma surgery device with the parameters and instrumentation specified in the table below, and to ensure the efficient functioning of the device throughout the duration of the contract. All costs of operating the device, including inspections, repairs, maintenance and servicing as well as training of the Ordering Partys staff in the use of the device, must be included in the price of the subject of the order in the scope of Part 2.
