1. The subject of the order is the provision of health services within the Ophthalmology Clinic and the Childrens Ophthalmology Clinic operating in the Specialist Clinic Complex of the City Hospital in Ruda Śląska Sp. z o. o. The term health services should be understood as all activities aimed at maintaining, saving, restoring or improving health as well as other medical activities resulting from the treatment process or separate provisions regulating the principles of their implementation. 2. Health services (including ophthalmological consultations for the needs of Occupational Medicine) will be provided during the Clinics working hours as needed in accordance with the Clinics working time schedule. 3.The doctor providing the services must have specialization in the field of ophthalmology. 4. A detailed work schedule is specified in the clinics work schedule applicable to the Ordering Party and is prepared in advance for each month. The ordering party reserves the right to change the schedule, but no later than 3 days before the start of a given month. The schedule is approved by the Head of the Specialist Clinics Team/Medical Director. 5. In accordance with the Act of May 13, 2016 on counteracting threats of sexual crimes, the contracting party will, during the examination of offers, check whether the Tenderer is not listed in the Register of Sexual Offense Perpetrators. If the offer of the Order Acceptor is included in such a register, it will be rejected. 6. Standards for the protection of minors are available on the Hospitals website at: www.szpitalruda.pl 7. Number of people providing services: 1 8. Each Bidder applying for the conclusion of a contract may submit only one offer.
