The subject of the contract is the delivery of medicinal products included in the list under Art. 266a, para. 2 of the ZLPHM, concluded on the basis of Art. 266a, para. 5, item 3 of the Law on Medicinal Products in Human Medicine, in conjunction with Art. 1, para. 1, item 2, art. 3, para. 3, Art. 15c, para. 1 and Art. 15b of Ordinance No. 10/17.11.2011 of the Ministry of Health, pursuant to which the Contractor accepts, after receiving a letter of consent from the Medicines Executive Agency (EMA) and a written request for the delivery of a medicinal product, to deliver the following medicinal product, in the quantities prescribed by UMBAL Sveta Marina EAD with a protocol - Annex No. 1 or No. 2, issued in accordance with Ordinance No. 10 of 17.11.2011, according to the CONTRACTORs offer, namely a medicinal product with an international non-proprietary name /INN/ - Carmustine and Trade name/pharmaceutical form/quantity of the active medicinal substance: Carmustin Waymade 100 mg powder and solvent for concentrate for solution for infusion x 1. The estimated delivery quantities are 5 packages, each containing 1 pc. vial in a package. The specified amount of the medicinal product can be delivered by one or several separate delivery requests, depending on the specific needs of the medical facility. The contracting authority is obliged to accept and pay only the quantity that it has requested and that has been delivered under the terms of the concluded contract.
